This notice explains how MyMedical Urgent Care collect personal information about you, how we use it and how you can inquire with us about it.

Who controls the use of your personal data
MyMedical Urgent Care Centre Ltd trading as My Medical Urgent Care is the company that provides the Laya health and wellbeing clinic service for minor injuries and minor illness. My Medical Urgent care is therefore the company that controls and is responsible for personal data that is collected in relation to your treatment in the Laya health and wellbeing clinics.
Who we are
When we talk about “we” or “us” or “our” we are talking about MyMedical Urgent Care.
We share your information only where necessary to help us provide and improve our services and to comply with regulatory and legal requirements. For further details see “Disclosure to Third Parties” below.
Our Data Protection Officer oversees how we collect, use, share and protect your information to ensure your rights are fulfilled. You can contact our Data Protection Officer by:

How we collect information about you
We collect information from you when you register at reception; enquire about the services we provide; complete registration and claim forms; use your credit card or debit card; and undergo a medical examination with us. We also collect information about you when you are referred to us by your medical practitioner or another organization; we also collect information through our CCTV footage and a patient satisfaction survey.
If the information we need is not provided, we may not be able to provide you with our services safely or at all.
How we protect your information
We keep our computers, records, and buildings secure. We also provide our employees with appropriate training. When you contact us to ask about your information, we may ask you to provide proof of identity. All of our measures help to protect your information.
How long we retain your information
To meet our legal and regulatory obligations, we retain your information for a period of time after you have contacted us or used our services. For further information regarding our retention periods, please contact our Data Protection Officer.
Our Legal Basis for Processing
To use your information lawfully, we use your information in accordance with at least one of the following legal bases:
•    For the performance of a contract
•    Legal obligation
•    To protect our and your vital interests.
•    Our legitimate interests; and/or with your consent.
To meet our regulatory and legal obligations, we collect some of your personal information for the provision of healthcare management.
How we use your information
We use information about you to:
•    provide our services to you.
•    identify areas where we can improve our service.
•    protect both our interests.
•    meet our legal and regulatory obligations; and/or
•    for the provision of healthcare management
Disclosure to Third Parties
Sometimes we share your information with third parties to:
•    provide our services.
•    analyze information.
•    collect debts.
•    sell whole or part of our business.
•    protect both our interests.
•    for the performance of a contract.
We expect and require these third parties to have the same levels of information protection that we have. We also have to share information with third parties to meet any applicable law, regulation or lawful request. When we believe we have been given false or misleading information, or we suspect criminal activity we must record this and tell law enforcement agencies, which may be either in or outside Ireland.
Your Rights
You have a number of rights in relation to your data, all of which apply in different circumstances:
•    Right of access to Personal Information - you have the right to receive a copy of the Personal Information we hold about you and information about how we use it. This right is applicable at all times when we hold your Personal Information (subject to certain exemptions).
•    Right to rectification of Personal Information - you have the right to ask us to correct Personal Information we hold about you where it is incorrect or incomplete. This right is applicable at all times when we hold your Personal Information (subject to certain exemptions)
•    Right to erasure of Personal Information - this right is sometimes referred to as ‘the right to be forgotten’. This right entitles you to request that your Personal Information be deleted or removed from our systems and records. However, this right only applies in certain circumstances.

Making a complaint
If you have a complaint about the use of your personal information, please contact the site at which we provided you with a service to let them know, giving them the opportunity to put things right as quickly as possible. If you wish to make a complaint you may do so in the following methods:
•    Verbally - In person or over the phone in our clinics.
•    In Writing- by email to dpo@mymedical.ie or by letter posted to MyMedical, Building 8, Cherrywood Business Park, Dublin 18, D18 W319

Please be assured all complaints will be fully investigated. We ask that you supply as much information as possible to help our staff resolve your complaint quickly.
If you are not satisfied with the response or with to contact our head office, please contact our Data Protection Officer.
You can also contact the Data Protection Commission in Ireland at www.dataprotection.ie.
Updates to this notice
We will make changes to this notice from time to time, particularly when we change how we use your information, and change our technology and products. You can always find an up-to-date version of this notice at our sites in which we provide our services.
